Additionally, i2i Systems is partnering with the National Association of Community Health Centers (NACHC) to capture additional statistics for the Million Hearts initiative, specific to hypertension.

The demonstrated success of Million Hearts is exemplified through the execution and adoption of i2i Systems Tracks and PopIQ product suite. i2iTracks ignites real-time, proactive care management by identifying patients at risk for Cardiovascular Disease, eliminating missed opportunities. i2i PopIQ delivers a cloud-based comparative analytics toolset that leverages multiple customers’ data sets to examine cross-population views into global population health management.

After few months of implementation, healthcare organizations across the country are reporting dramatic outcome improvements through the use of i2iTracks and PopIQ Million Hearts Toolkit. La Maestra Community Health Centers (LMCHC), which currently serves a 40,000 patient population, saw an 8% drop in missed hypertension and cardiovascular disease opportunities.

“That 8% shift is extremely meaningful; we want to make sure we address all the needs of our patients. When it comes to diagnosing, nothing is as fast and thorough as i2i Systems,” states Sonia Tucker, QI Director for LMCHC. With i2iTracks Million Hearts toolkit, LMCHC rapidly uncovered 453 patients that needed hypertension screening. Once those patients were recalled for a doctor appointment to determine diagnosis, 26.2% of them turned out to be hypertensive. “We saved over 120 lives using i2iTracks Million Hearts Toolkit,” says Tucker.

“We are changing outcomes and significantly impacting the stats around heart attacks and strokes. In the U.S., one of every three deaths is caused by heart disease or stroke, equating to more than two million heart attacks and strokes every year. In addition to the tragedy of losing lives to these two preventable causes, healthcare costs for these two conditions are $312.6 Billion,” stated Janice Nicholson, i2i Systems Co-Founder.

She continues: “These facts make us ask key questions: ‘How can we use big data to understand the extent of this problem?’ ‘How can we make a difference nationally?’ ‘Where are these patients and how can we bring awareness to the organizations seeing them?’ With PopIQ, we are starting to answer these questions on a much larger scale. Big data can tell us great stories about progress on initiatives like Million Hearts across millions of lives, so we can make a difference nationally.”

Having led the way for Million Hearts success for organizations across the country, Nicholson will be a featured speaker at next week’s NACHC’s Community Health Institute (CHI) and Expo in Orlando. The event is the largest and most important annual gathering of 2,000 community health center executives, clinicians, employees, board members and leaders. Her talk, “Become a Million Hearts Champion: Put Your Health Center on the Road to Hypertension Control,” will be delivered on Monday, August 24th, and is meant to propel additional health centers into Million Hearts participation and success.
