i2i Population Health, a national leader of population health management technology, will partner with Community Health Centers of Arkansas (CHCA) to provide population health management solutions to 11 Community Health Center organizations with a combined total of 76 locations serving 172,000 patients.

“Giving providers the tools they need to support population health and quality reporting is the mission of i2i,“ says Justin Neece, president. “We’re pleased to have CHCA leverage the grant they acquired to enhance their PHM strategy across the entire state.“ i2i will provide a population health management platform and develop metric reports to help CHCA with its quality improvement efforts. Previously, the centers had attempted to use a variety of electronic health record platforms without success.

“We are very pleased with our partnership with i2i and know this will give our Community Health Centers an excellent resource to access valuable data,” says Mary Leath, chief executive officer CHCA. “Having this real-time data will allow us to better serve our patients, understanding their needs and addressing those needs in the most effective, patient-centered manner possible. Partnering with i2i on a platform that works across Electronic Health Record systems will enable our Centers to continue to improve health outcomes and develop strategies that can help us achieve our mission of zero health disparity for all Arkansans.”

i2i was selected based on their rich history with Community Health Centers around the country and their dedication to getting the job done. CHCA was impressed with i2i’s problem-solving abilities and being able to fix any issues efficiently. The two entities will begin working to launch the population health management platform beginning August 15, 2016. Funding for the i2i platform comes from grants CHC AR received from the Health Resources and Services Administration and its Health Center Controlled Networks technical assistance grant program.
