Social Determinants of Health: Leveraging Population Health Technology for Risk Stratification and Care Coordination

This Clinical Excellence Case Study will showcase efforts by Siouxland Community Health Center to leverage technology for population health including creation of an innovative risk scoring methodology made possible by social determinants of health (SDoH) data from the PRAPARE Assessment Tool.

PRAPARE aligns with national initiatives including Healthy People 2020 and Uniform Data System (UDS). EHR templates exist for eClinicalWorks, Epic, GE Centricity, and NextGen, and i2i is currently developing a toolkit for collection and analysis of SDoH data with an estimated launch date in July.  Highlights from this case study include interventions for care coordination, treatment plan development and community resource connection to transform care for patients.
