(May 26, 2015) – i2i Systems, the leader in integrated Population Health Management and Analytics solutions for more than 15 years, has earned 2014 Patient Centered Medical Home prevalidation (PCMH) from the National Committee for Quality Assurance (NCQA) for its flagship product i2iTracks, the company announced today.

NCQA PCMH prevalidation includes a rigorous evaluation of a submitted health IT solution’s functionality including, but not limited to, reporting functions, report examples, screen shots, live demonstrations and other relevant documentation.

i2iTracks version 7.4 is now prevalidated by NCQA to receive autocredit points across a multitude of factors toward NCQA’s PCMH 2014 scoring. As well, it received practice support designation for key factors, which aid the practice in demonstrating compliance.

i2i Systems previously achieved NCQA PCMH prevalidation in 2011. Many of i2i’s clients have been recognized as Patient Centered Medical Homes, including Central Virginia Family Physicians (CVFP). The practice credits i2iTracks with giving them the tools they needed to do the tracking and reporting that is making a difference in the way they care for patients.

“We were simply unable to meet some of the requirements of PCMH without i2iTracks,” said Dr. Robert Elliot of CVFP, a group practice with over 100 providers, 219,000 patients, eight primary care offices, and two urgent care centers in the Lynchburg area of Virginia. CVFP achieved PCMH Level III recognition in 2014.
