Santa Rosa, Calif. (June 16, 2015)—The 2011 Patient Centered Medical Home model provided practices with the very basic framework to understanding Population Health. The 2014 PCMH model, encourages practices to begin the journey of managing the health of their populations and transforming to a value-based healthcare delivery system.
