[ssba]Santa Rosa, Calif. (November 19, 2015)—California-based i2i Systems, a pioneer defining Population Health Management and Analytics for the last 15 years, announced today that Michigan-based Family Health Center (FHC) has selected i2iTracks as their population health management solution.

A provider of care for over 45 years, FHC was awarded the largest Affordable Care Act grant for Federally Qualified Health Centers in the State of Michigan ($9.3m) in 2011 and has 7 locations in Kalamazoo County, 45 providers, and serves upwards of 30,000 active patients with over 95,000 visits per year.

FHC, like primary health care providers across the country, has seen a huge influx of patients as a result of the Affordable Care Act. Yet they have found a particularly high disease burden among the recently insured patients that have come to their clinic from medically underserved and low-income regions. FHC’s patients have higher rates of hypertension, diabetes, and asthma, and a perinatal mortality rate that is higher than the state average. In addition to chronic disease and maternal health, FHC has found greater demand for dental and behavioral health services and has recently focused on substance abuse, HIV/AIDS, and LGBT program development.

In addition to improving perinatal patient monitoring, i2iTracks will also be used to help FHC improve multiple HEDIS measures including appropriate medications for asthmatics and adolescents on multiple anti-psychotic meds. These patients have traditionally been difficult to monitor due to challenges with EHR data. With i2iTracks, FHC will proactively manage patients to better health.

For example, we know that diabetes is the leading cause of blindness in adults and with i2i Tracks we expect to be able to identify our diabetic patients who have not had that very important eye exam recently. We can use i2iTracks to identify them, contact them, get the eye exam scheduled, help us determine if they kept their appointment, and potentially prevent blindness in our patients,” shared Dr. Means.
