FRANKLIN, Tenn., and LITTLE ROCK, Ark., November 20, 2018 – i2i Population Health, a leader in actionable population health solutions, and ARcare, a rural community coalition serving Medicaid and Medicare populations in Arkansas, Kentucky, and Mississippi, released the results of a year-long PHM initiative.

It takes an engaged team with the right vision, the right setting and the right solutions.”

After recognizing opportunities to improve the quality and financial performance of their population health initiatives, ARcare leaders piloted a performance management program at KentuckyCare, one of its Federally Qualified Health Centers (FQHCs). Using i2iTracks® as the backbone, leaders identified clinical and outcome measures with the most significant impact on their population, then set out to close care gaps, improve outcomes, and generate sustainable revenues, enabling them to further their mission.

“Our results have been outstanding,” said Lauren Fields, ARcare’s Chief Care Coordination Officer. “In just 12 months, we increased diabetes and hypertension control by 29%, depression screenings by 90%, and HbA1c testing by 82%. We also increased revenues $1.3 million. It’s really been incredible. With our complex populations, i2iTracks has made population health simpler and has extended our reach into the communities we serve. It’s a fantastic tool and we rely on it tremendously.”

Given the accomplishments in Kentucky, ARcare has expanded the program to all of their health centers and integrated a rhythm of continuous improvement into the company culture. They are on track to demonstrate reduced cost of care for their ACO populations and are sustaining their revenue growth.
