FRANKLIN, Tenn., Sept. 13, 2017 — i2i Population Health, a national leader in population health management (PHM) technology and 2017 Best in KLAS category winner, welcomes KeyCare as the newest Health Center Controlled Network (HCCN) to implement i2i’s solutions. KeyCare and i2i have officially launched the implementation program that will greatly contribute to clinical and operational improvement at the sixteen participating New Jersey Community Health Centers (CHCs) in the current HRSA HCCN grant project. Five of the sixteen centers are currently using i2i and will benefit by deploying additional PHM solutions as being part of the network.

The relationship between i2i and KeyCare is effective as of July 31, 2017.
Since its establishment in 2001 by the New Jersey Primary Care Association, KeyCare has evolved into an autonomous Managed IT Products and Services provider offering a comprehensive suite of cost effective data management, reporting, compliance, planning, implementation and operational management solutions that maximize efficiencies at each of its Federally Qualified Health Centers (FQHCs). KeyCare, an HCCN, is owned by five FQHCs in the State of New Jersey and is currently responsible for leading the implementation of its second HRSA HCCN Grant Project, which includes eleven FQHCs who are not members of KeyCare. The network of sixteen participating Health Centers all receive the same benefits that assist them with providing outstanding services for their 398,000 patients at 96 sites. This HRSA grant funds the i2i initiative.
