PRiZiM®-powered dashboards Proactive Care and HRSA Compare equips payers, providers, and healthcare organizations with the capabilities to begin addressing health equity in their communities.

FRANKLIN, Tenn. – February 21, 2023 – i2i Population Health, the nation’s largest population health technology company serving the underserved, announces the release of two new dashboards, Proactive Care and HRSA Compare, to its advanced analytics pop health platform, PRiZiM®. Voice of Customer is an integral part of our product management program. This is another example of how i2i continues to innovate rapidly with our client needs.

i2i’s PRiZiM® is a cloud-based, advanced analytics and reporting platform for provider organizations, health networks or systems, and health plans. By aggregating disparate data sources into a single “source of truth”, PRiZiM gives users flexibility to analyze rich data sets via dashboards, reports, and analytics tools. PRiZiM® allows clients to automate quality measurement, implement timesaving care programs, and create on-demand reports to effortlessly track performance across the healthcare enterprise.

Powered by PRiZiM®, the two new dashboards will be:

  • HRSA Compare, which allows a user to see how their community health center, FQHC, compares to data submitted by other FQHCs to HRSA for the previous three years. Users can also compare their performance to individual states.

  • Proactive Care, which compares the current month’s performance of quality measures to the previous month to display and act on variances in compliance.

 

  • This instance of Proactive Care below shows compliance increases and decreases by home location. (Negative = percentage dropped) (Positive = percentage increased)

 

The main goal of the HRSA Compare tool is to give users the ability to compare their performance to other FQHCs to establish baselines. The image above highlights a specific FQHC on the left and all HRSA submitted data for a specific state. You can select a specific measure and compare it to regions or states, etc.

Additionally, Proactive Care gives an end user the ability to quickly see if any quality metrics for specified measures have changed (up or down) from the current month to the last calendar month. The purpose of this view is to allow users to catch potential compliance issues before they get out of hand.
